PT-141 (Bremelanotide) is a cyclic heptapeptide melanocortin receptor agonist derived from Melanotan II. It was FDA-approved in 2019 as Vyleesi for premenopausal women with hypoactive sexual desire disorder (HSDD) — making it the only peptide specifically approved for sexual dysfunction.
Unlike Viagra and Cialis, which work peripherally by dilating blood vessels, PT-141 acts centrally through the nervous system — specifically activating MC3 and MC4 melanocortin receptors in brain pathways associated with sexual arousal. This central mechanism means it works through desire and arousal rather than physical blood flow, and is effective in cases where PDE5 inhibitors are not.
PT-141 activates melanocortin receptors in the hypothalamus and limbic system — areas of the brain associated with sexual arousal and motivation. Activation of these receptors increases dopamine signaling in reward circuits, producing increased sexual desire and arousal.
This mechanism is fundamentally different from PDE5 inhibitors: PT-141 works whether or not there is physical stimulation and does not depend on nitric oxide signaling or vascular response. It addresses desire at the neurological level.
Phase 2 and Phase 3 clinical trials leading to FDA approval documented significant improvements in sexual desire, arousal, and satisfaction in women with HSDD. Studies in men show improvements in spontaneous erections and sexual satisfaction. The combination of PT-141 with oxytocin has been studied for additive effects on both arousal and emotional connection.
